We\u2019ll provide it within 30 days, and may charge a reasonable, cost-based fee.<\/p>\n\n\n\n<p><strong>Ask us to correct your medical record<\/strong><\/p>\n\n\n\n<p>You can ask us to correct your health information if it is incorrect or incomplete. We may say no, but will explain why in writing within 60 days.<\/p>\n\n\n\n<p><strong>Request confidential communications<\/strong><\/p>\n\n\n\n<p>You can ask us to contact you in a specific way (e.g., home or mobile) or send mail to a different address. We will agree to all reasonable requests.<\/p>\n\n\n\n<p><strong>Ask us to limit what we use or share<\/strong><\/p>\n\n\n\n<p>You can ask us not to use or share certain health information. We may decline if it impacts your care.
